Birth Registrations
Parents will submit birth registration information, either electronically through www.serviceontario.ca or on paper by mail, directly to the Office of the Registrar General.
Birth Certificates
Once the Statement of Live Birth is received and processed by the Office of the Registrar General, a Notice of Registration is forwarded to the parents. This could take up to 16 weeks from the date of birth. Once the Notice of Registration is received, a Request for Birth Certificate can then be submitted directly to the Thunder Bay office:

The Request for Birth Certificate form is available from the Vital Statistics Unit at Kingston City Hall or online at www.serviceontario.ca. This Web site also offers a fully interactive online application with help features to guide users easily through the application process.
