Public Information
Under secton 79 of the Social Housing Reform Act, 2000 and section 60 of Ontario Regulation 298/01 amended to 182/02 it is a duty of the Service Manager to provide public information on RGI assistance and Special Needs housing.
- The housing projects within the service manager's service area and the housing providers that operate them.
- How to apply for rent-geared-to-income (RGI) assistance.
- The eligibility criteria for RGI assistance.
- The requirements to be included in the special priority household category.
- The provincial and local occupancy standards in force within the service manager's service area.
- The requirements relating to waiting lists.
- The geographic areas into which the service manager has divided its service area for the purposes of its centralized waiting list.
- The rules and procedures in respect of the transfer of households to a unit in a housing project operated by a different housing provider.

The types of decisions of the service manager that are subject to internal review.
9.1 The rules and procedures of the service manager in respect of the internal review of decisions.
9.2 The rules and procedures of the service manager in respect of the opportunity for members of the household to comment on information that may form a significant basis for a decision that is adverse to the household.
- The alternative housing providers that provide housing within the service manager's service area to homeless or hard to house households.
- The criteria to be met for accommodation in a unit provided by the alternative housing providers under their mandates to provide housing to homeless or hard to house households.
- The mandates, established under section 99 of the Act, of each housing provider that operates a housing project within the service manager's service area.
- The housing projects within the service manager's service area that provide special needs housing and the supportive housing providers that operate them.
- How to apply for special needs housing.
- The eligibility criteria for special needs housing.
- The services offered by each supportive housing provider.
- The nature of the special needs housing provided by each supportive housing provider.
- The type of decisions of supportive housing providers that are subject to internal review.
- The rules and procedures of supportive housing providers in respect of the internal review of decisions.
- The rules and procedures of supportive housing providers in respect of the opportunity for members of the household to comment on information that may form a significant basis for a decision that is adverse to the household.
Members of the public are allowed to make copies of this information at their own expense.
All applicants for RGI assistance and Special Needs housing will be provided a copy of this information upon receipt of the application.
Community Housing Application (CHAP)
Former applications for housing issued by Housing Providers and the Housing Registry at 428A Barrie Street have now been replaced by a new
application form comprised of Part "A" and Part "B". Please discontinue use of the CHAP form effective October 1st, 2002.
