Information For Applicants

As of May 16, 2011, you may apply online for Ontario Works financial assistance through the Ministry of Community and Social Services using the Apply Online link at right.

HOW DO I APPLY FOR ONTARIO WORKS?

Or you can apply by contacting the City of Kingston's Community and Family Services Department at 613-546-2695, ext. 4980. You will be given an appointment time for an application interview and receive information about the Ontario Works Program. You will also be told which information and documents may be needed to complete the application process, for example:

  • Social insurance number;
  • Health number;
  • Proof of identity and date of birth;
  • Employment history/information;
  • Income and asset statements;
  • Shelter costs;
  • Status in Canada.

If you are not able to attend your application appointment, please contact the intake team to reschedule your appointment at 613-546-2695, ext. 4980.

After your Case Manager has received all of these documents, an eligibility decision will be made within four working days.

If you are eligible, your Case Manager will contact you and you will receive in the mail:

  • A drug card;
  • An explanation of the cheque; and
  • An income reporting statement.

Payments are deposited directly into your bank account.

A participation agreement is part of the application for financial assistance. This agreement:

  • Outlines your employment goals;
  • Is a step-by-step plan to help you reach these goals;
  • Lists employment related activities agreed upon by you and your case manager;
  • Will bring you closer to financial independence.

Speak to your Case Manager about financial supports for which you may be eligible while working towards your employment goals.

CONTACTING A CASE MANAGER  [Top]

An appointment is required to see a case manager. To leave a message for your Case Manager, please speak clearly and leave one message with:

  • Your full name, Member ID number, or your date of birth;
  • Your reason for calling;
  • Your telephone number or contact number;
  • The best time to reach you from Monday to Friday, 8:30 a.m. to 4:30 p.m.;
  • Calls will be returned within two business days.

Remember:

  • Report income monthly using the Income Reporting Statement;
  • Report any changes in situation (for example, living arrangements or income);
  • Contact your case manager when you have questions or concerns;
  • By participating in employment-related activities you will be closer to financial independence.
